If (patient name) cannot have a tolerance to these basic activities while in hospital THEN he/she is at risk for COMPLICATIONS such as skin breakdown, decreased tissue perfusion and muscle atrophy which could lead to a prolonged hospital stay Related to: bed rest, generalized weakness, imbalance between oxygen supply and demand (choose one) Outcome: Mr. Complains of weakness dx: Activity Intolerance *A tolerance to Activities is a basic physiological need. Patient stated he felt a little stronger and did not have any shortness of breath. Oxygen maintained at 2 liters per nasal cannula. Student nurse assisted him to ambulate to the bathroom and back proving two rest periods. Patient stated he feels weak and gets short of breath during ambulation. ![]() Patient in performing the activity Evaluation: Mr. Patient with ambulation * Providing assistance with ambulation will decrease energy expenditure and support Mr. ![]() Administer/Maintain oxygen at two liters per nasal cannula (if applicable) * Administering / Maintaining oxygen will allow for an increase in oxygen levels which will lead to an increased ability to tolerate activity. Provide rest periods * rest periods will allow for prevention of air hunger, to help with exertion Intervention 2. ![]() ![]()
